Certified Diabetes Educator RN Tilley Drive

Position Summary:

The Certified Diabetes Educator is responsible for the clinical coordination of care related to a specific group of patients or aspect of patient care.  Coordination of care extends across the organization and to the outside community.  S/he applies specialized knowledge through teaching of patients and family, consultation, research and guidance of other members of the health care team.  The ofganizations Standards of Patient Care and Nursing Practice guide the delivery of nursing services by this individual. The position demonstrates dedication to professional growth and development and to the continuous improvement of patient care. Maintains specialty certification, and maintains a minimum of 18 continuing education credits/certificate of attendance hours. Performs one formal presentation each year.

Education:

Registered Nurse, currently licensed in Vermont.  Baccalaureate degree in Nursing preferred.  Certified Diabetes Educator certification with 12 months. Minimum of 18 continuing education hours/year.

Experience:

Minimum of 3 years relevant clinical experience in diabetes.  Incumbent must work in an ADA Recognized Education Program or other nationally recognized diabetes education program.  Completion of one formal presentation per year.  BLS Certification to be obtained within 12 months.
