Care Manager
Hillside Children’s Center has an opening for a Care Manager in Syracuse, NY. This position is Full Time - 40 hours - Monday through Friday, with some flexibility. There is a possibility of weekend hours based on client or program need.
The Care Manager has responsibility for working as a lead member of the care team using principles of assessment, care planning, service and community referrals, and evaluation to coordinate wraparound services using person centered strategies for referred participants. This is a new position/program for Hillside. The caseload will be up to 50 active cases.
Qualifications
High School Diploma required, Bachelor’s degree in Human Services or related preferred. At least 2 years of Human Services experience with youth and families required. CASAC and/or Significant Experience with Housing/Homeless resources is preferred. Direct and significant knowledge of Oswego and/or Cortland Counties is preferred. Applicants must have a valid New York State driver’s license and must agency driving and insurance standards.
