Assesses plans, intervenes, monitors, evaluates and coordinates the options and services required to meet the patient's health needs while promoting quality, cost effective outcomes.
- Graduate of an accredited school of nursing
- BLS
- 1 year of experience required
- RN license within the state of NJ; graduation from an accredited school of nursing. Five years clinical experience in a behavior health setting. Preferred: Care coordination, utilization management, case management or discharge planning experience. Psychiatry and/or Chemical Dependence certification. BSN; managed care experience
